Hugely Impactful Partnership

In May this year, we launched our new partnership with the OneBridge health service, funded through Beddown.

Working alongside inCommunity teams, including the InCommunity Connect Assertive Outreach Program, we knew this program would fill a much-needed health response gap in the #Ipswich Community.

What we didn’t realise was just how impactful this partnership would be.

In National Homelessness Week, Beddown continues to raise awareness of the impact of homelessness, in particular for people sleeping rough.

We understand that agency for people sleeping rough is incredibly important. This translates to how a person may wish to navigate health support for medical issues and other support services that are offered to them when experiencing homelessness.

We now know that this program is both welcomed by our community and is delivering hugely impactful results. Swipe through to see the results achieved via this partnership.

OneBridge Partnership with Beddown National Homelessness Week Graphic 87 consultations for primary health care, both acute and chronic disease management. 57 incidences - Health Literacy: highly significant to understand diagnosis, symptoms and outcomes of health care and autonomous health navigation. Community says "Thankful for the service", "This is so cool", "Grateful the nurses are here". 47 Incidences - Mental Health: significant need for mental health support, brief intervention and referrals. 35 Incidences - Prescription (medication): requirement for OneBridge nurse practitioners (greatest need are antibiotics) and discussions around contraindictions and system management. 30 Incidences - Wound Care: High need for management of acute and chronic wounds as well as providing free dressings. 29 Incidences - Referrals: referrals to specialists and local services to grow health access and support.
